(1) Between 1 January 2017 and 30 June 2017, how many agencies were advised that their grant agreement with the Department of Health would cease when the current agreement expired on 30 June 2017?

(2) Please list the agencies and/or programs whose grant agreements with the department ended on 30 June 2017.

(3) How much notice were these agencies given of the cessation of their grant agreement funding?

I refer to WorkSafe’s inspection programs for 2016 – 17, and ask:

 (a) how many inspections did WorkSafe undertake;

 (b) of those outlined in (a), how many were:

 (i) proactive, spontaneous, with limited or without notice;

 (ii) routine; and

 (iii) initiated in response to a complaint;
